InfoEd – Delegate Request Form

Assigning a delegate to your InfoEd User Profile allows you to authorize another individual to view, edit or approve system records on your behalf. Provide the information below to add up to three delegates to your account. Additional delegates can be added by resubmitting this form as necessary.

*If granting approval authority, you must select delegates who are authorized to substitute for the roles outlined in the policy on internal approval of research related submissions. Note that these individuals will not receive routing notification e-mails in their UConn e-mail accounts. Your delegate(s) will have to monitor your InfoEd internal message box to view proposals or protocols that are awaiting approval.
